    Stroke

    Stroke is the third most common cause of death in the US. Over 700,000 strokes occur every year of which 20% are due to carotid disease. The major risk factors for stroke include tobacco abuse, high cholesterol, hypertension, diabetes, and cardiac arrhythmias.

     

    Typical stroke symptoms include focal motor or sensory deficits affecting an arm and/or leg, loss of vision in an eye, or difficulties in the ability to understand or express speech.


    Carotid endarterectomy is performed to help prevent stroke in patients with significant carotid disease. The Asymptomatic Carotid Atherosclerosis Study (ACAS) evaluated patients with asymptomatic carotid disease. In patients with stenosis greater than 60%, there is a 53% risk of reduction of stroke with surgery as opposed to medical management.


    The North American Symptomatic Carotid Endarterectomy Trial (NASCET) evaluated patients who suffered from a stroke and demonstrated a benefit of carotid endarterectomy if the blockage is > 50%.


    Carotid endarterectomy is a very well-tolerated procedure and typically only requires a single night stay in the hospital. Most patients only require Tylenol for post-operative pain and discomfort. The procedure is performed through a small incision in the neck as follows:

    A - Expose the carotid vessels through a neck incision.

    B - Open the artery longitudinally from the common branch to the branch supplying blood to the brain.

    C - Manually extract the plaque.

    D/E - Close the artery with a patch then the neck incision.
